We strive to deliver care as it should be – with people right at the centre About the Opportunity As a Partner in the Community Local Area Coordinator, you'll work directly with individuals, families, and local networks to build connections and identify opportunities within the community. This role focuses on person-centred approaches that celebrate individual strengths while fostering independence and belonging. You'll collaborate with people to map their local area, discover hidden gems, and create meaningful relationships that enhance quality of life. Key Responsibilities Build authentic relationships with community members and local organisations Support individuals to identify their goals, interests, and aspirations Connect people with existing community resources and networks Facilitate introductions and encourage natural community connections Maintain accurate records and contribute to team learning What We're Looking For Strong interpersonal and communication skills Experience in community development, social work, or related field Genuine commitment to person-centred practice Ability to work flexibly and autonomously Current driver's licence and willingness to travel locally Understanding of diverse community needs and cultural sensitivity How to Apply Click 'Apply Now' to submit your application.
