Role details The Care In The Right Setting at the Mackay Base Hospital is seeking applications for a Nurse Practitioner to join their team on a permanent basis. Your opportunity The Nurse Practitioner demonstrates advanced and expanded clinical skills, knowledge and high-level capacity to translate these into contemporary evidenced based care for acute patients in HiTH or in a residential aged care environment. The Nurse Practitioner provides effective autonomous and accountable health care that incorporates application of high-level knowledge and skills at an advanced level of nursing practice, including assessment and management of clients to ensure safe and optimal care. This includes but is not limited to; the direct referral of clients to other health care professionals; prescribing medications; and ordering diagnostic investigations. It is the purpose of this role to provide expert knowledge and clinical skills, utilising a case management model, and coordinate the comprehensive care of patients for HiTH, GEMR-HiTH, Interim care and RaSS consumers. The role increases available support for residents to receive care in the Residential Aged Care Facility (RACF)'s where it is safe to do so, and the resident agrees to it. Reviews the patients within the RACF at GEDI, CNC or medical staff request, who would otherwise be sent to an Emergency Department. Reviews are supported by the provision of inpatient hospital standard acute care substitution within the RACF to those patients meeting appropriate criteria. Has a special interest in care of older persons. The role increases support to individuals in custody at the Mackay Watchhouse where it is safe to do so. Reviews the patients within the Mackay Watchhouse at QPS or medical staff request who would otherwise be sent to Emergency Department. About the Team The Care in the Right Setting (CaRs) program aims to improve access to health care and reduce the demand on hospitals by improving flow and promoting the right care in the right time for the right outcome. The CaRS Team includes Hospital in The Home (HiTH), Geriatric Evaluation Management / Rehabilitation - Hospital in The Home (GEM/R-HiTH), Residential Aged Care Facility Support Service (RaSS), Mackay QPS (Watchhouse) and Geriatric Emergency Department Intervention (GEDI) and four hospital funded beds at Glenella Care Residential Aged Care Facility awaiting permanent placement (INTRM). Patients receive quality care that is tailored to their individual health care needs.
