Your responsibilities Deliver face-to-face and occasional tele-support across clinic, community and home settings Coach clients (and their carers) living with disability or complex/chronic conditions in behaviour-change, participation and goal achievement Design, implement and evaluate person-centred care-plans tailored to each client’s goals and circumstances Provide targeted education for clients, carers and staff when required Communicate and collaborate effectively across our multidisciplinary team to ensure integrated, best-practice care Contribute new ideas and help us evolve and elevate our OT offering. Who we’re looking for A bachelor’s or postgraduate qualification in Occupational Therapy (or equivalent) Eligibility to work in Australia and registration (or near registration) with the relevant OT board Experience (or strong interest) working with adults/young adults with disability (NDIS, ABI, CP, neurological disorders, psychosocial) Comfortable working across multiple settings (clinic/community/telehealth) A collaborative mindset, a growth-orientation and a willingness to shape service-delivery Excellent communication skills — you make clients, families and colleagues feel heard, supported and engaged.
