Your opportunity The purpose of this role is to provide expert knowledge and clinical skills, utilising a case management model and coordination of comprehensive care of patients for the Connected Community Pathways program. About the Team The Aboriginal and Torres Strait Islander Health Unit is an essential element of the Mackay Hospital and Health Service support services and ensures the development and delivery of culturally appropriate services and models of health care to Aboriginal and Torres Strait Islander people and families.
