Your opportunity The Senior Outreach Pharmacist service encompasses continuum of care disease management in a rural generalist context including but not limited to cardiovascular, endocrinology, maternity, orthopaedic, neurological, oncology, paediatric, respiratory and surgical conditions. These services will be delivered in local and outreach inpatient and outpatient-based service facilities, to assist in the implementation and maintenance of the Australian Pharmaceutical Advisory Council (APAC) guiding principles to achieve continuity in medication management.
