ABOUT THE ROLE: As a Clinical Case Manager, you’ll be the trusted advocate and care director for our high-hour clients. You’ll lead the onboarding journey, conduct in-home assessments, develop tailored care plans, and ensure seamless service delivery. You’ll be the primary point of contact for clients, supporting their evolving needs and ensuring their experience reflects our brand promise. Key responsibilities include: Conducting initial intake assessments and developing holistic care plans Leading annual reviews and ongoing updates based on changing client needs Managing incidents and liaising with internal and external stakeholders Supporting conflict resolution and understanding client funding Acting as a brand ambassador during client visits and consultations ABOUT YOU 2–3 years' experience in case management, ideally in aged care or community care Clinical qualifications (RN or EN) highly desirable Strong assessment, documentation, and care planning skills Exceptional communication, negotiation, and interpersonal abilities A proactive, agile mindset with a commitment to continuous improvement A current driver’s license and reliable vehicle What's in It for You?
