Position: Case Manager, First Response Team Classification: SCHADS Crisis Level 2 Department: Social Mission – Homelessness, Nambour Employment: 12-month Contract | Full-Time Why this role matters The Salvation Army (Salvos) is one of the largest providers of homelessness services across the country. Our philosophy is that every person should be afforded dignity and respect and that no one should be without a safe, affordable, and secure home. We provide services and supports to adults, and families with accompanying children in the form of accommodation, case management, assertive outreach, financial assistance, community connection and referral services. About the role Join us as a Case Manager and make a real difference for individuals and families at risk of homelessness. Provide compassionate, trauma-informed, person-centred support that empowers resilience and lasting change. Be part of a dedicated team delivering hope and tailored care across South-East Queensland. Provide over-the-phone and face to face intervention, triage, interim response and case management to individuals and families throughout the region. Includes initial assessment, appropriate referral with development and implementation of case plans that achieve set outcomes. How you will make an impact Ensure that delivery of services and support is aligned with the National Homelessness Stream Model of Care. Establishing relationships with individuals who access the service, that encourage choice, self-determination and participation Liaise daily with First Response team members to provide collaborative and comprehensive response to client needs Working within an ethical decision-making framework to administer brokerage funding where appropriate in consultation with Team Leader Ongoing assessment of the individuals’ presenting issues, vulnerabilities and needs from initial intake to exit Working within an ethical decision-making framework to administer brokerage funding where appropriate in consultation with Team Leader Case Management is provided at the office or in Community in a Mobile Support Capacity. Engagement in continuous quality improvement activities, including, but not limited to: audit processes, data discovery sessions, reports, Most Significant Change Methodology Participates in local network meetings inclusive of Communities of Practice meeting, working groups and projects Displays high levels of emotional intelligence, with an awareness of how personal values and beliefs can impact work What you will bring Degree qualification in social work, welfare, community development or related field OR demonstrated high level of sector knowledge and skills attained through previous appointments, service and study/development A degree level qualification is preferred.
