The Opportunity We’re seeking a full-time Care Partner to join our Home and Community Services team in TOOWOOMBA, QLD. In this rewarding role, you’ll deliver person-centred, high-quality home care through assessment, care planning, monitoring, and review — helping our clients maintain their independence and wellbeing at home. Reporting to the Clinical Care Manager, you’ll work closely with clients, families, and internal teams to ensure every service reflects McLean Care’s values and quality standards. Key Responsibilities Conduct client assessments and develop individualised care plans Coordinate and monitor service delivery to ensure quality and compliance Maintain accurate and timely client documentation Collaborate with rostering and admin teams for smooth service delivery Advocate for client wellbeing with dignity and respect Identify additional support needs and facilitate appropriate referrals Participate in WHS training, meetings, and quality initiatives Contribute to community engagement and continuous improvement Key Selection Criteria Essential Qualifications Certificate III in Individual Support OR Certificate IV in Ageing Support OR Enrolled Nurse (Diploma of Nursing) Current First Aid and CPR certification Unrestricted driver’s licence and reliable, insured vehicle Essential Skills & Experience Experience in assessment, referral, or coordination within aged/community care Excellent communication and interpersonal skills Ability to work both independently and as part of a team Strong documentation and computer literacy skills Understanding of WHS, Duty of Care, and Aged Care Standards Genuine commitment to person-centred care and McLean Care’s values Desirable Knowledge of CHSP and Consumer Directed Care Familiarity with Aged Care Quality Standards Experience coordinating services for older clients in the community What We Offer $43.05 per hour super allowances Salary packaging up to $18,900 net (boost your take-home pay) Blue Light Card for retail and service discounts Employee Assistance Program (EAP) for wellbeing support Annual flu vaccinations & wellbeing programs A supportive, respectful, and values-driven culture Apply Now If you’re passionate about supporting older Australians and want to make a real difference in your community, we’d love to hear from you.
