We’re preparing for the transition to the new Support at Home program, and this role will play a key part in ensuring clinical excellence, safety, and innovation as we navigate this exciting change. The Opportunity We’re seeking an experienced Community Clinical Coordinator to join our passionate team. In this role, you’ll lead clinical coordination for clients with complex care needs, working closely with Case Managers, Registered Nurses, Allied Health, Care Workers and external providers to ensure safe, high-quality, and person-centred care. You’ll be the clinical lead in developing care plans, supporting continuous improvement, and mentoring Bene’s nursing team in best practice and compliance. Key Responsibilities Conduct comprehensive clinical assessments within a wellbeing and reablement framework. Develop and review complex care and behavioural support plans. Provide clinical leadership, supervision, and guidance to Registered Nurses. Provide clinical guidance to home support staff providing direct care to clients. Participate in incident investigations, clinical audits, and quality improvement initiatives. Support Case Managers in coordinating multidisciplinary care and service referrals. Deliver staff education and clinical credentialing (e.g., medication competency). Maintain accurate documentation and reporting in line with legislation and Bene’s Quality Management System. Build strong relationships with clients, families, and service providers to enhance care outcomes. About You You are a compassionate, proactive professional who thrives in a collaborative environment and enjoys mentoring others while maintaining clinical standards and compliance. Essential Criteria Registered Nurse with current AHPRA registration Demonstrated experience in community, home care, or aged care settings Strong understanding of wellbeing, reablement, and restorative care Excellent communication, documentation, and time management skills Ability to work both autonomously and within a multidisciplinary team Current National Police Check and Driver’s Licence Desirable Previous experience in a similar community coordination role A second language is advantageous Understanding of Italian culture Why Join Bene Work with a supportive, values-driven team that genuinely cares about people.
