Intervention Expertise: Utilise a broad range of evidence-based occupational therapy interventions. Goal-Directed Care Planning: Collaborate with participants and their support networks to establish clear, measurable goals and develop a comprehensive care plan to achieve them. Equipment & Home Modification Prescription: Assess the need for assistive technology and recommend appropriate equipment or suggest home modifications to enhance independence and safety. Chronic Illness Management: Support participants in managing chronic illnesses by developing strategies for daily living activities and promoting self-management skills. Empowering Education: Provide education, advice, and recommendations to participants and their families, equipping them with the tools and strategies to manage their conditions and promote self-reliance. Collaborative Communication: Develop strong relationships with participants, families, carers, and other service providers, fostering effective communication and collaboration for optimal outcomes. Comprehensive Documentation: Maintain accurate and timely documentation, including assessments, progress reports, and NDIS/Aged Care reports. Here's what makes a great fit for this role: Qualifications & Clearances: Hold a completed Tertiary qualification in Occupational Therapy and be registered with the relevant body. Plus, a current driver's license and the ability to obtain Working With Children Checks, National Criminal History Check and NDIS Worker Screening Check (if not already acquired).
