About the Role South West Healthcare (SWH) is seeking applications for a Mental Health Clinician to join our Mental Health Referral Services - Centralised Triage team. In this vital frontline role, you will conduct mental health screenings and connect individuals to the most appropriate services whether this is specialist mental health support or referrals to other providers — ensuring they receive the right care at the right time. As a Mental Health Clinician in this role, you will work at the heart of mental health care in a fast-paced, collaborative environment where every decision matters. You will triage urgent cases, coordinate follow-up care, manage bed allocations, and maintain accurate records all while working closely with a dedicated multidisciplinary team. Key Responsibilities Conduct mental health screenings and gather relevant information Triage and allocate care according to priority and need Manage urgent cases and liaise with relevant teams Maintain accurate records and documentation Communicate referral outcomes and coordinate care pathways Support quality assurance processes and compliance requirements Position overview Employment Type: Full Time, Ongoing 80 hours per fortnight with a monthly accrued day off Location: Warrnambool Campus Classification: Registered Psychiatric Nurse Grade 3, Social Worker/Occupational Therapist/Psychologist Grade 2 Salary (pro rata) plus superannuation Registered Psychiatric Nurse Grade 3: $100,932.00 - $112,091.20 Occupational Therapist Grade 2: $88,030.80 - $101,197.20 Social Worker Grade 2: $88,030.80 - $101,197.20 Psychologist Grade 2: $94,478.80 - $106,906.80 What You Bring Degree in Occupational Therapy, Social Work, Psychology, or Nursing. Experience in mental health, ideally in triage. Strong communication skills. Ability to work effectively in multidisciplinary teams. Knowledge of relevant legislation and best practices. Membership/registration with AASW or AHPRA (as relevant).
