About Allevia Limited: Allevia is a registered NDIS Provider, providing support to people who have an intellectual disability. Many of the people we support have a dual disability - along with their intellectual disability they also have a physical disability, and/or mental illness. About the Role: This role is a great opportunity for someone who already has experience in the disability sector and is ready to take the next step. You’ll be supported by experienced coordinators while gaining hands on skills to grow your career as a Support Coordinator. What you’ll be doing: Supporting participants to understand and implement their NDIS plans. Assisting with coordinating services and supports to meet individual needs. Building and maintaining positive relationships with participants, families, and other providers to ensure quality service delivery. Promote independence and empowerment. Maintain accurate participant records, case notes, individualised plans and preparing documentation and reports to support NDIS Plan reviews. Foster relationships and collaborate with other professionals, community organisations, and stakeholders to enhance support options and resources available to participants. Advocating for participants to ensure their goals, preferences and needs are met. Learning from and working alongside senior Support Coordinators. What we’re looking for: Strong interpersonal, communication and organisational skills. Someone enthusiastic and eager to learn OR someone with Disability experience A caring, person-centred approach. Ability to work well in a team and follow guidance. Willingness to grow in the role with training and mentorship. Current NDIS Worker Screening Check, Working with Children Check, Police Check, First Aid Certificate, and Driver’s Licence with access to a reliable vehicle Minimum: Cert IV in Disability, Community Services, Mental Health, or equivalent.
