The intake and assessment team are responsible for initial contact, safety and needs assessment, client data entry and scheduling of initial appointments with the relevant program, supported on referrals and inclusive service planning and follow up as appropriate. Joining a values driven, supportive and inclusive team you will: Provide professional support, advice and information to staff and stakeholders about services in Centacare NQ and the broader community to ensure people are informed and supported with choices around services Work collaboratively with senior leadership and program management across the organisation to ensure the provision of targeted and holistic care to Centacare NQ clients Act as a central point for referrals and service responses for external stakeholders such as Queensland Police Service and Centacare NQ’s online enquiries. Actively participate in the coordination and delivery of information sessions about Centacare NQ services to build the profile of the organisation and promote accessibility of the community to services Coordinate cross program service delivery for clients engaged with multiple Centacare NQ programs to ensure client outcomes can be supported through timely service coordination. Coaching, mentoring, and motivating team members to achieve program goals, including assisting with induction of new staff and supporting staff in case discussions. Take initiative in the review and refinement of intake and assessment functions, policies, and procedures. Maintain oversight of databases including managing data integrity and support to management in relation to data reporting training for staff. Skills And Experiences To be successful in this role you will need: Possession of a relevant degree in Social Work, Psychology or Behavioural Sciences. Demonstrated experience and knowledge of working in a client triage and/or intake service, including the ability to undertake risk assessments relating to suicidal ideation, domestic and family violence and child protection concerns with the ability to liaise with other agencies to ensure client safety. Excellent interpersonal and communication skills including the ability to work effectively to network and collaborate with a wide range of professionals, both internal and external, for the purpose of creating the most effective referral pathways. Experience guiding a team to service excellence Ability to sensitively step in when required and nurture team dynamics. A current Driver Licence. Currently hold, and/or eligibility to obtain and maintain, both a Disability Worker Screening and a Working with Children Blue Card.
