Senior Helpers Far North Queensland is a family owned and operated business. We are looking for a compassionate, dedicated and driven case manager to assist our clients with community care case management. This is a full-time role located in Cairns, QLD. Monday to Friday . Key responsibilities include: Working with local Support Coordinators, Allied Health professional and clinical teams to meet, engage and build relationships with clients to develop, refine and achieve their goals. Provide information and support to clients and their families/carers to monitor the use of their funds. Ensure accurate case noting, service agreements and client reviews that reflect forward planning in line with the participant's funding plans. Conduct annual reviews for all clients - revise care plans, goals, service provisions to quality standards and budget expenditure and maximization. Ensure service provisions are conducted ethically, fairly and within the statutory legal and contractual requirements at all times. Compliance with Health policies and procedures to ensure a safe working environment and detailed, timely incident reporting. General office administration duties. The successful applicant will have: AHPRA Registration Understanding of NDIS and Home Care Packages. Previous case management experience. Demonstrated knowledge of the both community care funding models. Exceptional customer service, negotiation and influencing skills. Excellent ability to connect with multiple stakeholders and think commercially. Strong interpersonal skills and high levels of resilience. Driver’s license and access to own vehicle, company car available for use in business hours Professional verbal and written communication skills.
