About your role: Deliver high level clinical pharmacy services within the Community Care team within the CHHHS in accordance with prescribed professional and ethical standards with infrequent need for direct clinical practice supervision. Contribute to the development of clinical practices, procedures that support the continuum of care engaging cross-discipline referencing where relevant in First Nations and Rural and Remote Care. Liaise with patients and their families, other health professionals, supervisors, students and researchers, as required to ensure continuum of care.
