About the opportunity As part of our expanding Support at Home team, you will be responsible for ensuring a high level of quality clinical care is provided to our clients living in the community. Responsibilities include, but are not limited to: Clinical assessments Liaising and collaborating with multidisciplinary team members Wound management Monitoring the progress of client’s goals, needs and preferences in collaboration with the Support at Home Coordinator Managing clinical incidents Any other duties as required within scope of practice Roster: Monday, Wednesday,Thursday and Friday 10am - 6pm Tuesday 8:30am - 5pm About you The ideal candidate will be an Clinical/Registered Nurse with experience in Aged Care or a similar industry. You will have highly developed clinical and critical thinking skills to efficiently coordinate and deliver clinical support, guidance and care as required in collaboration with the client, their families and other health professionals. You must have a current Australian driver’s licence and current AHPRA registration. To be considered for this role you will have Current AHPRA registration Current Australian driver’s licence A reliable vehicle that will pass a Vehicle Safety Checklist A current First Aid certificate or we can organise At least 6 months experience caring for the elderly in their own homes preferred but not mandatory You must also be willing to obtain a National Police Certificate before commencing employment as well as a functional assessment.
