Transitions is a voluntary c ounselling and intervention program for young people on Department of Child Safety orders within the Morayfield and Caboolture areas. The program focuses on promoting stability, engagement with family, education, and community, and improving overall quality of life. We are looking for a Senior Case Manager to provide therapeutic case management and advocacy for young people under Child Safety orders. In this role you will: Deliver counselling, advocacy, and mediation support. Facilitate group and individual interventions to build resilience and life skills. Work closely with families, carers, and Child Safety to support stable placements. Encourage positive engagement with school, work, and community. About You: You are degree-qualified in Human Services, Social Work, Psychology, or a related field, with experience working with young people in out-of-home care or under Child Safety orders. You bring strong therapeutic, mediation, and advocacy skills and can build strong relationships with carers, families, and service partners.
