Due to the nature of this role travel will be required across the Western Brisbane region. The role As a Clinical Care Coordinator, you will work closely with your care manager to ensure care team members are providing high quality care to clients within a set geographic across community services. Having an engaging & collaborative communication style you will easily build relationships & links within the community with your clients, staff & wider networks ensuring a personalised service is delivered to each client. Holding an excellent understanding of industry standards & clinical indicators you will ensure that all staff are educated & supported in delivering care needs, with the ability to highlight any areas of improvements.
