We have an immediate need for an experienced General Practice Nurse for a busy eastern shore practice. You will work with a friendly team to provide primary healthcare support, including patient assessment, immunisations, wound care, chronic disease management, and patient education. The role requires a patient-centred approach, cultural sensitivity, and the ability to work collaboratively within the practice team. Key responsibilities include: Clinical Care: Conducting health assessments, screenings, and immunisations. Providing chronic disease management, including care planning and patient education. Assisting GPs with minor procedures and wound care. Reviewing and discussing test results with patients. Patient & Practice Management: Providing patient education on various health topics. Coordinating patient care plans and referrals to other specialists. Managing and ordering clinic supplies and maintaining equipment. Updating and maintaining accurate patient records and health information. Collaboration & Communication: Working collaboratively with doctors and other healthcare professionals to provide integrated care. Liaising with hospitals, community agencies, and allied health professionals to coordinate patient care. Acting as a patient advocate and providing culturally appropriate care. Essential Skills & Knowledge Strong clinical assessment and health promotion skills. Proficiency in wound care, immunisations, and chronic disease management. Knowledge of clinical and practice management software. Excellent communication and interpersonal skills for patient education and teamwork.
