Coach and support Connectors; run short weekly huddles and case reviews Build relationships with assessors and providers; organise case conferences as needed Keep data tight: same-day notes, simple workflows, on-time monthly reporting Pay & benefits: SCHADS L4 super generous salary packaging (PBI) Option 2: Elder Care Community Connector (SCHADS Level 3) Walk alongside Elders and families to get support started quickly and safely.
