Description The opportunity: We are growing and expanding our Social Support Groups and are looking for Social Support Workers to join us. This is an exciting opportunity to join a team of passionate professionals who provide participant led group activities, outings and events while caring for the individual’s goals. You will play an important role in facilitating social support groups, offering one-on-one assistance and helping create a safe, welcoming and supportive atmosphere for all participants. Responsibilities include but are not limited to; Planning with participants/team and implementing a range of quality activities. Providing transport using work cars and bus (12 seater). Providing personal care support. Assistance with meal service as required. To be involved in review of care plans, progress notes, discussions with clients and family and update progress notes at the end of the day. Make contributions to the team. Skills And Experiences Our ideal candidate: Certificate III in Individual Support (Aged & Disability) or equivalent. Current Full Victorian Driver’s Licence. Food Safety Certificate or agreement to complete. First Aid Certificate Level 2 or agreement to complete. The ability to work with and support participants with a range of diverse needs. This may include people with dementia, isolated older people, people with disabilities. Ability to be creative, flexible and innovative in the delivery of a quality social support group program. Promoting the independence of our participants, their choices and actively involving them in the group program. Experience working well within a team setting. Strong communication and interpersonal skills. Experience using various IT applications and processes. Ability to drive a 12-seater bus and agreement to complete training.
