Lead and coordinate the implementation of a consistent intake process and referral pathways across community health services. Deliver direct and indirect person-centred nursing care ensuring safe, quality, and evidence-based practice. About you: You are an experienced and compassionate clinical nurse consultant, skilled in community nursing, leadership, and driving service improvements.
