The Role An opportunity has become available for a Nurse Practitioner (Cognitive Disorders) to join Gold Coast Health (GCH) where the focus of our 12 000-strong team is on providing world-class health care to people living in the Gold Coast, and northern New South Wales region of Australia. You will Provide advanced knowledge in gerontology, cognitive disorders, and dementia care Collaborate with stakeholders to provide patient care to people living with all forms of dementia Complete comprehensive assessments and provide diagnosis of cognitive disorder Provide clinical support, mentorship, and supervision to potential Nurse Practitioner candidates About you To be successful in this position you will require: Bachelor of Nursing or equivalent Registration with the Nursing Midwifery Board of Australia Evidence of vaccination, or be able to be vaccinated against and remain vaccinated against - Measles, Mumps, Rubella, Varicella, Pertussis and Hepatitis B Tuberculosis screening, to identify if the tuberculosis vaccination may be required Declaration of serious discipline history must be disclosed Criminal History Screening will be required for new employees (GCH will cover cost) Benefits Flexible work options Career Development Salary Packaging Modern Facilities Diverse Work Culture Research Opportunities 12.75% super 17.5% leave loading Salary Permanent part time (60 hrs p.f.) Base salary ranging from $75.28 - $78.59 per hour About Us The Cognitive Disorder Clinic is a community-based service within the Chronic Disease & Post-Acute Programs Team. The Cognitive Disorder Clinic is a diagnostic program, providing a comprehensive multi- disciplinary diagnostic and intervention service. In conjunction with forming a medical diagnosis, the clinic provides nursing and allied health input to support the social and psychological care of managing dementia for clients and their families. The Cognitive Disorder Clinic works closely with the Community Dementia Service which sits within Community Frailty Service. The Community Dementia Service is a nurse led service that assists and supports people living with dementia and their careers in decision making. The Service provides referral and triaging, comprehensive assessment and case management, post diagnostic support and information, initial care planning and referral to other agencies, as required, ongoing support and care planning, prevention of unnecessary hospital admissions, medication review, transition from hospital to home including home visits, advance care planning, carer education and support.
