The Role An opportunity has become available for an Enrolled Nurse (Complex Care) to join Gold Coast Health (GCH) where the focus of our 12 000-strong team is on providing world-class health care to people living in the Gold Coast, and northern New South Wales region of Australia. Base salary ranging from $35.92 - $38.13 per hour. About Us The Integrated Multidisciplinary Post-acute and Complex Care Team Services comes under the overarching leadership of Integrated Care Services/Rehabilitation, Aged Care and Community Services. Incorporated into IMPACCT Services, the Multi-disciplinary Avoidancee and Post-acute Service provides a 7-day a week service which provides care in the community in the vital two weeks following hospital intervention to enable seamless transition of care for discharged patients. In particular, those who would otherwise have to wait longer in hospital for allied health interventions or up to two weeks at home before receiving ongoing community service from the Non-government Organisation sector. MAPS also focuses on identifying and providing care to community dwelling individuals who would otherwise be at risk of presenting to the acute hospitals through direct referrals form General Practitioners and the Queensland Ambulance Service.
