About The Opportunity An exciting opportunity for a Breast Cancer Survivorship Nurse at SWSLHD working with GPs and SWSLHD Oncology Services to provide shared care and follow-up care for breast cancer patients. What You'll Be Doing The Clinical Nurse Consultant (CNC) is an expert practitioner who, through leadership, facilitates evidence-based practice. The CNC will promote a service which fosters clinical excellence, research and professional leadership. The CNC contributes to quality health care by collaborating with health care providers and consumer groups, developing clinical practice standards and evaluating their outcomes. The CNC is responsible for providing services which facilitate effective management of the care in the given specialty. Responsibilities Provide leadership in clinical practice and facilitate evidence-based care in breast cancer survivorship. Collaborate with GPs, Oncology Services, and multidisciplinary teams to plan and deliver follow-up care. Develop and evaluate clinical practice standards and outcomes within the specialty. Contribute to quality health care and ongoing professional development within the service. Support patient-centered care and effective care management for breast cancer patients. Qualifications Current Authority to Practice as a Registered Nurse with AHPRA. At least 5 years full-time equivalent post-registration experience, with at least 3 years in the specialty field and approved postgraduate nursing qualifications in a relevant field (or equivalent). Demonstrated capability to interact professionally and resolve conflicts when they arise. Evidence of strategies to maintain current knowledge of clinical practice advances and research in the specialty. Ability to communicate effectively with patients/consumers, carers and the multidisciplinary health care team to identify and respond to care needs.
