Available as full or part time, you’ll work as part of a multi-disciplinary team as the singular Social worker to support and empower customers in navigating their social and health challenges, enabling them to lead healthy, independent lives while maintaining connections to their community. As part of this role, you will work across all metropolitan regions, ensuring our services reach and support individuals in a variety of settings. Key Responsibilities: Provide Social Work services within a community setting, focusing on empowering customers to make informed decisions about their care. Conduct assessments, monitor progress, and review care and support plans to ensure customers’ needs are met. Apply a wellness and reablement approach to service delivery, supporting customers' overall wellbeing. Collaborate with a multi-disciplinary team to achieve optimal health outcomes for customers. Work closely with internal and external stakeholders to design services for customers' individual needs. Maintain accurate documentation and record-keeping in compliance with best practices and regulatory standards. And much more! About You: • Bachelor’s degree in social work and eligibility for membership with the Australian Association of Social Workers (AASW). • Previous experience working as a Social Worker for a minimum of 3 years. • Proven ability to empower customers to make decisions and provide advice on available services. • A strong problem-solving approach and ability to deliver services through assessment, goal setting, and evaluation. • Excellent relationship-building skills and the ability to work collaboratively with others. • Knowledge of aged care and older person’s health and needs. • Strong documentation skills with a high degree of accuracy. • A satisfactory police clearance for working in Aged Care (less than 12 months old) is required before commencement. • Only applicants with relevant experience in a community or health setting will be considered.
