We are seeking a highly organised and experienced Home Care Advisor to join our Support at Home Team. About the role The primary objective of the role is to plan, coordinate and provide day-to-day administration of Home Care Packages; ensure the activities of the Home Care Packages operate effectively and efficiently to meet the identified needs of frail elderly people & their carers. Key Responsibilities Your main responsibilities will include, but not be limited to: Build and maintain strong professional relationships with clients and their caregivers. Engage in community outreach and networking to attract new clients through referrals, seminars and events. Guide clients and their supporters in navigating aged care services, entitlements, and available resources. Conduct initial and ongoing assessments to develop and review personalised, goal-oriented care plans based on clients' needs, motivation, and preferences. Develop, implement, monitor and review of service user's care plan in consultation with the client. Provide person-centred case management through home visits and coordinated support with families, carers, and health professionals to ensure quality, holistic care. Managing financial aspects of service users’ budgets, including monthly statements. To coordinate the services and activities of the Support at Home Packages and to provide direct care when necessary. Making referrals, attending case conferences, providing information and support to service users/carers and obtaining their feedback. Manage client incidents and feedback promptly, ensuring timely resolution and continuous quality improvement. Ensure that all complaints are reported, investigated and resolved promptly and appropriately. About the Successful Applicant The successful candidates should possess but not limit to the following: Essential: Minimum 3 years of working experience in aged or community-based services for older people Tertiary qualifications in aged, disability, allied health and social work, or related areas e.g. Nursing, Community, Health, Welfare, or attained Certificate IV in Care Support Services / Aged Care, case management Demonstrated ability to work independently and work well with others Excellent interpersonal and time management skills Demonstrated knowledge and application of WH&S, EEO and anti-discrimination legislative requirements Speak fluent English, and able to communicate in both Cantonese and Mandarin Strong report-writing skills in English First Aid Certificate or willingness to obtain one Current full driver's license and a reliable, fully insured vehicle Able to pass employment checks, including a National Police Check clearance Unrestricted work rights in Australia Computer literacy, including knowledge and competency in intermediate Microsoft applications Desirable: Experience in Home Care Services/dementia care/respite care Experience in case management Experience in leading a team Experience working in a community-based setting Experience in working with Culturally and Linguistically Diverse populations Good understanding of the new Aged Care Act and Strengthened Aged Care Quality Standards Benefits We offer a range of benefits including competitive pay rate, ongoing training and career development opportunities within the organisation.
