Purpose of Position The Clinical Care Coordinator is responsible and accountable for supporting the Registered Nurses and Care staff in the clinical management of the facility, the delivery of clinical quality care, maintenance of standards and promotion of a positive work environment. The Role: You will be working 0830-1630 every Tuesday & Wednesday every second Monday (37.5 hours per fortnight) Responsibilities Include Monitor the care plans of residentsAct as an adviser in response to the complex nursing and care needs of all residentsPractice continuous improvement activities to maintain a high standard of client careMonitor the implementation of care plans and regularly review and update as appropriateManage each resident’s condition and progress, and ensure that documentation is regularly evaluated in accordance to SCC guidelines and accurately reflects ongoing careLiaise as required with Doctors, Paramedics, hospitals, health care professionals and other specialist service providers Requirements 5 years’ experience in caring for the aged or health related areaRegistered Nurse experience Strong leadership skills Good interpersonal and written English communication skills Ability to play an active and positive role as part of our Care Team, providing quality care and support to frail aged people Numeracy skills – reconciliations and statistics Sensitivity to and understanding of the needs of the elderly in a residential care setting What’s in it for you?
