About the Role We are seeking a Flexi HCP Care Partner to join our growing team. This role is designed for someone who thrives on variety and adaptability, providing coverage across multiple localities and teams as needed. You will step into different client caseloads and office environments, supporting continuity of care when colleagues are on leave, when workloads peak, or during periods of organisational growth. Key responsibilities include: Delivering person-centred care coordination and support for HCP clients across multiple localities. Providing interim or relief coverage for caseloads, ensuring seamless service continuity for clients and their families. Conducting assessments, reviews, and intakes of new clients when required. Ensuring services are delivered in line with Aged Care Quality Standards and HCP compliance requirements. Building strong, professional relationships with clients, families, colleagues, and external stakeholders in a variety of settings. Supporting the broader team with flexibility and a problem-solving mindset. This is a mobile role with travel across Melbourne (North, West, South and East). While based out of our Carrum Downs office, you will work across multiple sites with some WFH flexibility. About You You are a supportive, adaptable, and highly organised individual who enjoys stepping into dynamic environments. You thrive on variety and can quickly build trust with clients and colleagues. You also have: Experience in a similar role and relevant qualifications. Knowledge and experience in Aged Care, Disability, and/or Home & Community Care. Strong communication and interpersonal skills, with the ability to foster meaningful relationships quickly.
