The SURe Care and Recovery Coordination program offers trauma-informed, wraparound support tailored to each person’s journey - whether online, in the community, or face-to-face. Your role, your impact As a Harm Reduction Clinician, you’ll be at the heart of our outreach and care coordination efforts—working with high-risk populations to reduce harm, promote recovery, and connect people with the supports they need. Key responsibilities include: Deliver brief interventions, psychoeducation, and relapse prevention strategies Provide harm reduction services including safer use education, overdose prevention, and distribution of NSP and Naloxone Conduct comprehensive assessments across AOD, mental health, physical health, and social domains Develop client-centred care plans and facilitate warm referrals to treatment and health services Engage high-risk populations through outreach and community-based initiatives Support clients in transitional housing with regular check-ins and care coordination Maintain accurate records and contribute to service improvement Collaborate with multidisciplinary teams and external partners Who we’re looking for Experience in AOD and mental health sectors, ideally in harm reduction roles Strong interpersonal skills and trauma-informed practice Ability to manage crisis situations and engage diverse communities Minimum qualification in AOD Skill Set Current CPR certification MARAM training or willingness to undertake upon commencement Current driver’s license without limitations and ability to transport clients What will make you stand out Certificate IV in Youth Work, Social Work, or Advanced Diploma in AOD Competencies in dual diagnosis (completed or in progress) Experience working with justice-involved individuals, CALD communities, or people experiencing homelessness What’s in it for you At Each you’ll be supported to be your best.
