As a Rehabilitation Consultant/Social Worker you will manage a portfolio of clients with a physical injury or psychological disorder, completing case management and reports to facilitate return-to-work outcomes. You will develop the skills needed to conduct motivational interviews with clients as well as influencing stakeholders to achieve the best possible outcomes for your clients Within your role, you will: Manage a portfolio of clients with a psychological or physical injury Identify barriers preventing clients from returning to work Organise appropriate intervention with various stakeholders Monitoring the progress of clients by maintaining regular contact Compiling regular case records Writing return-to-work plans Assisting individuals to understand and resolve barriers by providing information Attend medical appointments with your clients Complete reports after assessments and meetings Communicate with medical professionals regarding diagnosis and treatment Monitoring the progress of individuals you are supporting Facilitate a safe and timely graduated return to work Document case notes as part of reporting and compliance requirements About You Current registration with ASSW - essential Min one years’ experience in a similar role Bachelor’s degree in Social Work Car and valid Driver’s license Police check or willingness to obtain a police check prior to commencement Ability to build strong stakeholder relationships Pinnacle Rehab is an equal opportunity employer who encourages applications from suitably qualified candidates from all cultures, ages, religions and genders.
