Job Description Young Mothers for Young Women (YMYW) provides peer and professional support to young pregnant and parenting women (25 years and under), their children, and families living in the Brisbane City Council region. Through our Wellspring Hub, young women can access targeted family support, group programs, antenatal services, child health clinics, and GP services. This two-generation program supports both parents and children, celebrating the strengths of young mothers while addressing interconnected challenges such as housing, health, social isolation, employment, education, and income through an integrated approach. We are seeking a compassionate Family Support & Advocacy Worker to provide targeted, trauma-informed outreach and case management to young women and their families with high and complex needs. This role focuses on improving family functioning, housing stability, and community connections, while navigating systems such as child protection, health, education, and domestic and family violence services. This is a permanent full time employment opportunity at Level 4 of the SCHADS Award. Desired Skills and Experience Key Responsibilities Provide targeted outreach, home visiting support, coordinated case management, and advocacy to young pregnant and parenting women (25 years and under), their children, and partners. Conduct holistic family assessments, develop and implement support plans, and connect families with community resources to improve family functioning, housing stability, and community connections. Support positive parent–child interactions, promote school readiness, and assist with accessing antenatal, postnatal, and child health services, including through partnership with Mater Mothers’ Hospital. Provide advocacy, safety planning, and referrals for young women experiencing domestic and family violence, and assist families to navigate housing, child protection, and other key systems. Essential Criteria: Relevant tertiary qualification and experience in a similar role. Understanding of the issues faced by young pregnant and parenting women, their children, and partners. Experience providing targeted family support through assertive outreach and home visiting. Ability to assess needs, document plans, and work collaboratively within multidisciplinary teams. Desirable: Demonstrated knowledge and experience of working with families in relation to children’s development. An understanding of the service system responding to young families in the Brisbane City Council area.
