About the Role : We are seeking a compassionate and proactive Community Worker to provide support to people with disabilities and their families. This role involves assessing individual needs, developing tailored support plans, and working collaboratively with a range of services to deliver positive outcomes. You'll be part of a team committed to advocacy, empowerment, and social inclusion. Key Responsibilities : Work directly with clients to identify their needs and design personalized support, education, and training programs; Conduct interviews and assessments to understand the challenges clients are facing; Track client progress and provide ongoing feedback and documentation; Connect clients with other support services and external agencies when additional help is needed; Identify gaps in community services and contribute to improving local support systems in areas such as health, housing, employment, and education; Build relationships with community organizations, government departments, and businesses to raise awareness and advocate for inclusive services; Support families and care for individuals with disabilities in group homes, adult service centers, and institutional settings; Provide mentorship and guidance to young people navigating personal, emotional, or financial difficulties; Supervise individuals on probation or parole, offering rehabilitation-focused support; Contribute to funding proposals, program reports, and communications with stakeholders.
