Key Responsibilities : Coordinate and monitor delivery of services under Home Care Packages (HCP)/Support at Home Client management, monitor care plans, client outcomes Support intake, onboarding for new clients Manage and support a team of community care workers Liaise with families, GPs, allied health professionals Ensure compliance and quality as per standards Optimise budgets and resources To be successful : Experience working in Home/Community Care as a Case Coordinator or Home Care Manager Experience in managing and coordinating Home Care Packages HCP/CHSP or NDIS Excellent engagement and communication skills Amazing customer service, friendly approach, compassion and values focused Current driver's licence National Police Clearance What you can expect in return : $128,800 package $16K Salary Pkg Laptop Phone $10,400 Remote Allowance Company Vehicle provided Housing - fully subsidised Build lasting relationships with clients and represent the company Amazing organisation with leaders who value your contribution!
