SUPPORT FACILITATOR, THE WAY BACK SUPPORT SERVICE FIXED TERM FULL-TIME CABOOLTURE Your Next Role! RFQ is seeking a dedicated and values-driven Support Facilitator to join The Way Back Support Service, operating across the Redcliffe–Caboolture catchment area—including Deception Bay, Woody Point, and Bribie Island. This is a full-time, fixed-term position based at our Caboolture office. The position is responsible for providing short term support facilitation services (non-clinical care and practical support) to people who are at risk of suicide. The Way Back Support Service has been developed by Beyond Blue and is delivered to people who have been admitted to a hospital following a suicide attempt or people experiencing a suicide crisis. The program is a psychosocial support service, delivered in the form of community outreach; focusing on supporting and empowering clients to build resilience and respond to crisis with the available supports within their community. No day is the same, but it may include: Promoting a supportive environment grounded in values such as trust, compassion, empowerment, and respect, while encouraging personal growth, conflict resolution, and lived experience. Delivering 1:1, outreach-based, non-clinical, psychosocial support to members of the community who have experienced recent suicide attempts or suicidal crisis. Providing recovery-oriented support in line with RFQ’s Models of Practice, including capacity building, community connections, and collaborative development and review of Wellbeing Plans and safety strategies. Conducting risk assessments using the SIDAS and K10 tools, participating in case reviews, and escalating client or organisational risks appropriately. Working with internal teams, hospitals, and external services to ensure continuity of care and support clients’ transitions and connections to health and community services. Maintaining accurate, timely records using digital systems, and ensure clear, respectful communication within the team and with stakeholders. Engaging in supervision, performance reviews, and continuous improvement efforts, contributing ideas and staying informed of policies and best practices. You will need: A Diploma or Certificate IV in Community Service (or related field), or significant proven experience supporting individuals living with mental illness or navigating crisis, including suicidal ideation. A NDIS Workers Screening and Blue Card. Understanding of suicidal behaviours, suicidal crisis/ideation and its effects on individuals, families and the community. Ability to develop and implement support plans in collaboration with clients, while adhering to organisational policies, procedures, and standards. Strong teamwork and communication skills, with the ability to contribute positively to team goals and advocate effectively for clients and the organisation. Demonstrated ability to work collaboratively with internal and external stakeholders to achieve client and organisational outcomes. Effective time management and proficiency in using office and mobile technology to complete tasks, maintain records, and support service delivery. Why RFQ?
