As a Tier 1 Aged Care On‑site Pharmacist (ACOP), you’ll be an integral part of partnered residential aged care facilities (RACFs), working side‑by‑side with care teams to deliver safe, effective, and resident‑focused medication management. Key Responsibilities Conduct regular medication reviews and reconciliation, especially during transitions of care Collaborate with GPs, nurses, and RACF staff to resolve medication-related issues Lead and participate in multidisciplinary case conferences and GP rounds Advise and report to the Medication Advisory Committee (MAC); assist in establishing MACs where needed Implement quality use of medicines (QUM) initiatives, including audits and deprescribing strategies Provide culturally safe, person-centred care that respects resident preferences and values Educate RACF staff on medication protocols, high-risk medicines, and regulatory compliance Maintain accurate documentation and contribute to continuous improvement processes. Qualifications Required To be eligible for Tier 1 ACOP placement, you must meet the following criteria: Pharmacy Registration- Current registration with the Pharmacy Board of Australia Credentialing- Completion of ACOP Tier 1 training modules (via PPA or approved provider) Vaccination Status- Up-to-date with all RACF-required vaccinations (e.g., influenza, COVID-19) Police Check- National Police Clearance suitable for aged care work Professional Indemnity Insurance- Valid and current coverage Experience & Skills Ideal candidates will bring: Minimum 2 years’ experience in community pharmacy or clinical pharmacy setting Prior exposure to aged care, medication reviews (e.g., RMMRs), or HMRs preferred Vaccinating pharmacist ideal.
