We are seeking a highly experienced and passionate Registered Nurse Clinical Coordinator to join a dedicated healthcare team in Sydney. This role offers the opportunity to provide leadership and support in delivering exceptional client care, while coordinating clinical services across NDIS, HCP, and private care programs. The successful candidate will be client-focused, resilient under pressure, and capable of managing complex clinical cases while leading a multi-disciplinary team. Key Responsibilities Provide clinical and people leadership, supporting and leading a multi-disciplinary team. Assess, develop, review, and deliver Clinical Care Plans. Act as the primary point of contact for all program-related enquiries. Liaise with internal and external stakeholders, including referrers and allied health professionals. Utilise clinical expertise to assess, evaluate, and implement appropriate care interventions. Train, mentor, and support care workers as required. Ensure compliance with accreditation standards (ACHS, NDIS), external legislation, and internal policies. Contribute to Clinical Coordinator meetings and audits, both internal and external. Promote a strong customer service focus and culture of compliance. Maintain clear, accurate, and up-to-date client care plans and documentation. Actively contribute to business growth by supporting service quality and billable hours targets. Requirements Current AHPRA Nursing Registration (Essential). Minimum 2–3 years’ post-registration nursing experience. Demonstrated leadership and management capabilities. Experience supporting clients in the community, including NDIS and HCP clients. Strong clinical knowledge of Aged Care legislation and compliance standards. Exceptional communication and interpersonal skills. Ability to work effectively in a fast-paced, team-oriented environment. Sound computer literacy (MS Office, Excel). Current Police Check and Driver’s Licence.
