As a Care Community Lead, you’ll provide clinical and operational leadership across our Care Community guiding your team to deliver person-centred care that promotes wellness and reablement. You’ll be trusted to make a valuable impact by: Lead, mentor and develop a high-performing multidisciplinary team, including Clinical Care Partners, Assessment Partners, Care Partners, and Service Partners. Ensure services meet program guidelines, quality standards, and customer budget requirements while aligning with organisational strategy. Champion a restorative, wellness and reablement approach to care, ensuring customer outcomes are central to every decision. Build and maintain strong working relationships with Community Aged Care leadership, external community care and health providers, and customers/supporters. Drive continuous improvement, quality, and risk management processes to enhance service delivery and staff capability. What you’ll bring To thrive in this role, you will have: Bachelor of Nursing tertiary qualification. Current registration and practising certificate with AHPRA (or equivalent). Proven leadership experience in a community aged care or health service setting. Senior case management and/or coordination experience with a strong customer service focus. Excellent knowledge of aged care legislation, standards, and programs. We would also love you to have: Experience applying wellness and reablement principles in care delivery. Ability to lead through change, foster a learning culture, and achieve KPIs. Strong interpersonal skills to engage and advocate for people from diverse backgrounds.
