The Role: As a Care Partner you will be responsible for managing the entire lifecycle of Home Care Packages (HCP) and Support at Home (SaH) while providing excellent customer service for consumers and adhering to the Aged Care Standards. You will onboard and provide oversight of a caseload of Doutta Galla consumers through your comprehensive assessment skills. Furthermore, you will utilise your extensive knowledge of Commonwealth Home Care Packages to optimise a consumer's home care journey. Key Areas of Responsibility: Monitor, review and adjust care plans in response to changing needs Conduct eligibility assessments and develop and maintain individual care plans and budgets, supporting care recipient choice and participation Provide care coordination assessment and care planning Liaise with service providers and monitor care recipients’ wellbeing, and making referrals to complementary community programs Assist care recipients in selecting services that align with their aged care assessment and collaborates with them to determine how these services will be delivered Maintain accurate timely documentation and comply with clinical governance safeguarding and mandatory reporting requirements Efficient time management for handling multiple clients and tasks Proficiency in Microsoft (Word and Excel); knowledge of Alayacare is highly desirable. The successful candidate will have the following experience and skills: You will be a confident, friendly, approachable, and professional individual who will maintain the relationship and communication with the Clients and their families Experience with or a strong understanding of the HCP/SAH Guidelines, the Aged Care Act, Aged Care Quality Standards, and My Aged Care Minimum 3 years of recent experience in community-based aged care and a formal qualification in relevant area of health, e.g. Diploma in Nursing, CertIII Individual Support (Ageing), CertIV Aged Care, Diploma Community Services (Case Management) Demonstrated experience in the provision of case management and home-based assessments Demonstrated experience in managing client feedback and complaints Experience in identifying and implementing service improvements Demonstrated experience in budget monitoring Excellent oral and written communication skills.
