The Exciting Opportunity: Manage and oversee a caseload of Home Care Package clients across Perth’s north and east. Lead delivery of high-quality, client-focused clinical services, ensuring effective risk management within funding and compliance guidelines. Conduct holistic assessments, develop and update care plans, and communicate with clients, families, service providers, and colleagues. Provide clinical leadership and governance to ensure safe, quality care consistent with policies, government regulations, and strengthened quality standards. Develop and implement strategies to manage quality and risk associated with clinical and care services. Be accountable for the organisation’s safe and effective infection control processes as the Infection Prevention Control Lead (IPC). Ensure compliance with all legislation, best practices, and internal policies to achieve excellent client outcomes, and participate in the on-call roster as required. About You: Registered Nurse with current AHPRA registration Experience working as a Clinical Nurse within a Home Care or Residential Care setting Previous clinical case management experience (including assessments, care planning, compliance, and budgeting) essential. Experience in Dementia and end of life care Strong working knowledge of aged care legislation Confident managing complex caseloads, with strong clinical and interpersonal abilities.
