About the role Join our team in an exciting role as an Allied Health Professional - Physiotherapist, where you will make a difference to our community clients to help them regain their independence and optimise quality of life. You will work with clients 1:1 in their homes and in group setting to provide rehabilitation and engagement for older people through person-centred model of care. As part of our dynamic Allied Health team, you will help shape the direction of Allied Health services within our organisation. Collaborate closely with our supportive team, including the Allied Health Team Leader, other Allied Health Professionals, Home Care Manager and Coordinators, and Clinical team members in both Residential, Community and Rehabilitation. You will work alongside other health team members to understand client care needs, plan interventions, and review outcomes, supporting residents and clients to achieve their goals. What You’ll Do: Conduct physiotherapy assessment and develop individualised goal-based treatment plans Support clients in achieving their goals towards independence and safe living at home Provide clinical support to the Community Transitional Care and Community program teams and function as an escalation point Provide oversight of external provider’s clinical assessments and equipment prescription Contribute and deliver Falls Prevention and relevant Health Promotional Activities to promote Healthy and Positive Aging What You’ll Bring: Relevant qualification in an Allied Health profession (Physiotherapy) with demonstrated experience in area of expertise Current registration with relevant registration authority as per legislation Ability to provide education, support and coaching to direct care employees, clients and their families Well-developed communication (written and face-to-face) and interpersonal skills to liaise with clients, carers families and direct care employees Knowledge of the aged care quality standards (desirable) Special Requirements This is a NDIS risk-assessed role – a current NDIS Worker Screening Clearance is required Recommended to have current vaccinations in line with government health directives (e.g. COVID-19, influenza) Must complete a pre-employment medical assessment as part of the recruitment process South Australian Drivers Licence Why Join LHG?
