About Hummingbird House Hummingbird House, Queensland’s only children’s hospice, operates as a medically supported ‘home away from home’ for children affected by a life limiting condition, and their families . The stand-alone purpose-built facility offers 8 guests in care beds, 5 family accommodation suites and a unique ‘end of life’ suite. Underpinned by the notion of creating a ‘warm embrace’ for children and their families, the multidisciplinary team provides 24/7hr scheduled and emergency specialist care short break stays; pain and symptom management; care at the end of life; and psychosocial support programs for the whole family . With strong connections locally, nationally and internationally with specialist paediatric palliative care services, Hummingbird House is an integrated community-based service delivering flexible child and family centred care. About the Role The Enrolled Nurse is responsible for providing holistic care in the guests’ home, that is an extension of the care model provided at Hummingbird House and meets the individual needs of the family. The Enrolled Nurse will embrace the philosophy of Hummingbird House, working closely with nurses and personal care workers. The Enrolled Nurse will provide hands on care to Guests within Hummingbird House and in a community setting. Key Role Accountabilities: Align one’s workplace behaviour in accordance with the Wesley Mission Queensland Model of Care, the Wesley Charter and Wesley Mission Queensland’s Vision, Mission and Values. Deliver holistic care and support to guests and families accessing the service, following relevant care plans and under the supervision of the clinical team. Observe, monitor, interpret, document and report signs, symptoms and changes in the child's status. Medication administration. Contribute to the formulation and implementation of care plans. Assist in the orientation of new staff . Participate in activities with guests as and outlined by relevant practitioners. Deliver care which is family centred, evidence based and meets all statutory requirements. Support guests with all aspects of daily living, including but not restricted to, personal care, associated light domestic duties and social/community participation. Provide guests with a physically and emotionally secure environment and promote their physical, intellectual, emotional, social, cultural and spiritual well-being. Facilitate access to appropriate family supports in consultation with the Family Support Coordinator. Act promptly should any changes to a guest’s condition be observed , seeking advice from the relevant clinical staff as appropriate . Undertake delegated care tasks for guests and families as directed by the clinical team. Work within the Human Services Quality Framework. Work effectively as part of the Hummingbird House and as part of a wider team throughout the organisation, including close consultation with the Nurse Unit Manager and the Family Support Team. Work within the NDIS Quality and Safeguards Commission. About You Current Enrolled Nurse registration with the Australian Health Practitioner Regulation Authority (AHPRA) with no restrictions. Ability to provide high level nursing care within the Enrolled Nurse Scope of Practice. Demonstrates passion and commitment to connecting the community to the mission and vision of Hummingbird House. The ability to maintain confidentiality, discretion and professionalism ensuring that the dignity and rights of guests and families are always respected . Proven high level teamwork and collaboration skills to achieve objectives and promote positive working relationships. Enthusiasm and highly effective interpersonal, written and oral communication skills. Understanding and commitment to the principles on continuous quality improvement. Experience in supporting people with illness and complex and/or chronic care needs/disability is essential. NDIS Worker Screening Clearance held before commencing . NDIS Worker Orientation Module ‘Quality, Safety and You’ completed before starting.
