Remuneration: Band 3 - $73,605.21 - $88,997.93 superannuation (pro rata) generous NFP salary packaging options up to $18,550 Part time (0.6 FTE) maximum term role until 12 December 2025.- (Choose your preferred working days) Docklands office location - Hybrid; work arrangement available You will be part of the Health Systems Integration team, working closely with HealthPathways team members, clinical editors, and diverse internal and external stakeholders to ensure the organisation meets its goals for timely, high quality service integration and digital platform improvement. You will coordinate the planning, execution and delivery of HealthPathways projects working across teams. This includes managing key initiatives such as provider directory audits, broken link resolutions, and service mapping, while building collaborative partnerships with stakeholders to enhance integration, quality, and continuous improvement in local health service pathways. For more information on Staff benefits, please visit: https://nwmphn.recruitmenthub.com.au/Staff-Benefits/ What will my typical week look like: Project manage small scale HealthPathways projects against timelines and finite resources, recognising barriers and risks, while suggesting solutions Network with internal and external stakeholders to support development and implementation of pathways and ensure integration of GP resources and information Collect and maintain local service provider information Map public health services and identify services gaps to inform the NWMPHN Population Needs Assessment Assist in the commissioning processes as required Prepare a range of reports and other documentation to support the HealthPathways program to meet reporting requirements of the organisation Relevant Resources: Reconcilation Action Plan NWMPHN Governance Framework NWMPHN Strategic Plan 2024 - 2028 About you: Relevant qualifications in project management, public health, health promotion, health service management, public administration, clinical studies, or a related field Demonstrated project management skills e.g. experience in scoping, planning, monitoring, and reporting on projects Knowledge and understanding of the Victorian health service system and contemporary health policy issues across the acute and primary care interface Excellent oral communication and interpersonal skills including an ability to interact effectively and appropriately with a wide range of varying stakeholders and collaborators.
