Position Vacant: Case Manager – Homelessness Services Full Time Classification: SCHADS Award – Crisis Level 2.1 Location: Spring Hill 38 Hours/week | Monday – Sunday (flexibility available) Applicants must be available to work shifts at times between 6am - 8pm and during weekends . We provide services and supports to adults, and families with accompanying children in the form of accommodation, case management, assertive outreach, financial assistance, community connection and referral services. About the role: We are seeking a passionate and client-focused Case Manager to join our dedicated team delivering support services within the National Homelessness Stream Model of Care. This is a full-time role that plays a key part in supporting vulnerable individuals and families on their path to stable housing and wellbeing. The Case Manager works closely with clients to develop and implement client-led case plans, ensuring service delivery aligns with national and state strategic priorities and frameworks. You will work to empower individuals, build resilience, and support sustainable outcomes. How You Will Make an Impact: In this role, you will: Deliver services in line with the National Homelessness Stream Model of Care. Build effective, trust-based relationships with clients that promote self-determination and active participation. Provide case management support in line with our model of care and best practice principles. Conduct ongoing needs assessments from intake to exit to ensure responsive and flexible support. Collaborate with clients to develop individual case plans with targeted interventions to meet their unique needs and goals. What You Will Bring: A degree in social work, welfare, community development or a related field Demonstrated experience in a social services environment, preferably working with people experiencing homelessness highly advantageous Strong understanding of trauma-informed care, risk assessment, and culturally responsive practice.
