We are currently seeking a dedicated and experienced Community Worker to join our team based in Hoddles Creek, Victoria . About the Role The Community Worker will be responsible for delivering specialist support coordination services to NDIS participants, focusing on building their capacity, resilience , and connection to community and mainstream services . The role will involve proactive engagement during times of crisis , linking clients with appropriate services, and developing support and action plans that facilitate long-term outcomes. Key Responsibilities Provide specialist support coordination in a complex service delivery environment. Assist participants in understanding and utilising their NDIS plan funding. Coordinate services and liaise with stakeholders, including government and community organisations. Resolve crisis situations and support development of participant resilience. Coach and mentor participants through their NDIS journey. Assist with intake processes, rostering, and service planning. Monitor outcomes and prepare reports for NDIS plan reviews. Qualifications and Experience Diploma or higher qualification in Community Services, Social Work, Disability or related field (AQF Level 5 or above). Minimum 1 year experience in a community, disability, or case management role. Strong knowledge of the NDIS and local support networks. Excellent problem-solving, communication, and project coordination skills. Valid Working with Children Check and Police Clearance. Current driver's licence.
