This role is ideal for a motivated individual with a strong foundation in family support and a passion for empowering individuals to live independently and meaningfully. Key Responsibilities Work collaboratively with clients to understand their individual needs and develop tailored educational, support, and training plans. Conduct client interviews and assessments to identify social, emotional, financial, or behavioural challenges. Monitor client progress and maintain accurate documentation and progress reports. Refer clients to relevant services and support agencies to ensure comprehensive care and assistance. Evaluate community needs and available resources in areas such as health, welfare, housing, employment, and training. Build strong relationships with community groups, government departments, welfare agencies, and private organisations to promote awareness and improve access to support services. Deliver support and care to families, children, and individuals with disabilities in various settings including group homes, adult service units, and community institutions. Supervise individuals on probation or parole, ensuring compliance with rehabilitation plans. Assist young people in navigating social, emotional, and financial difficulties by providing practical solutions and advocacy. Prepare funding proposals, resource requests, and reports for submission to government agencies and partner organisations. Requirements Qualifications : Associate Degree, Advanced Diploma or Diploma in related field in line with ANZSCO Skill Level 2 . Experience : Minimum of 1 to 2 years’ experience as an family support or community setting.
