You will also work with other health professionals to maximise the individual's physical and mental wellbeing. We are looking for experienced and dedicated Support Workers to provide assistance and support to individuals with complex needs. This role focuses on providing high intensity support to participants with complex and multiple support requirements, encouraging their independence and self-determination. Support participants to meet their goals particularly in alignment with their NDIS plan with a focus on safety, dignity, and client goals. Provide in-home support with health requirements, personal care and daily living tasks in alignment with the participant needs and the Care Team support requirements Assist with the delivery of activities to enhance the development of capacity and skills across physical, social, emotional, and intellectual domains Work with a team of health professionals, family, friends, and carers to implement a program of support including the implementation of Behaviour Support Plans and other Allied Health Plans Respond to crisis situations including behaviours of concern as listed in the Behaviour Support Plan Prepare meals and ensure dietary needs are met. Promoting independent living Optional and, where required, provide: Assistance with medication administration, as per policy, procedures and health professional directions Manual handling assistance - training will be provided when required Behavioural and Mental Health Support Provide 1:1 support to a client with complex needs, including seizure management (e.g., oral suction, medication administration) and personal care (e.g., hygiene, mobility).
