You’ll deliver evidence-based physiotherapy services to elderly clients under the Commonwealth Home Support Programme (CHSP), Home Care Packages (HCP), and other funding streams. This role is pivotal in helping clients remain independent and safe in their homes and communities. This is a fixed term contract role, with a preferred starting date in October. Conduct comprehensive assessments and deliver tailored interventions aligned with aged care quality standards Collaborate with clients to identify goals and develop care plans Provide group-based programs focused on cognition, balance, wellness, and reablement Liaise with internal and external stakeholders including My Aged Care, GPs, and case managers Maintain accurate documentation and meet billable hour targets Travel across aged care planning regions as required About you Tertiary qualification in Physiotherapy and current AHPRA registration Strong clinical assessment and report writing skills Excellent communication and interpersonal abilities Ability to work independently and manage time effectively Current driver’s licence and willingness to travel across East and North Metro Melbourne Valid Police Check and Australian working rights Why VMCH?
