Established in 1998, Katherine West Health Board is an Aboriginal Community Controlled Health Organisation that provides a range of clinical, preventative and public health services to clients in the Katherine West Region of the Northern Territory. With four major health care centres and a range of satellite centres, KWHB services an area over two times the size of Tasmania (162,000km²), helping to improve the health and wellbeing of some of the most remote communities in Australia. Position Description Physiotherapist Position Description Exercise Physiologist Position Description Dietitian Position Description Diabetes Educator Position Description Occupational Therapist Roles and responsibilities (but not limited to): Provide high quality assessment and support within your allied health scope for individual clients in the Katherine West region. Collaborate with other key members of the KWHB team, to ensure policies and practices are consistent with community priorities and preferences. Ensure integrated service delivery and follow up that is coordinated and communicated as required with health leadership, internal and external providers and visiting staff. Participate in preparing reports and adhere to timelines and reporting objectives as outlined in funding agreements. Support and participate in Quality Improvement activities within the Health Centre to enhance the safety and effectiveness of care. Provide follow up monitoring and care as directed through health assessment, TCA and/or General Practitioner Management Plan, coordinating as required with other relevant internal, visiting, and external providers.
