Description The Rural Locum Assistance Program (Rural LAP), a subsidiary of the multi-award-winning, Australian-owned healthcare provider, Aspen Medical, plays a key role in supporting the Australian Government’s rural workforce capacity agenda by delivering essential healthcare services to rural and remote regions. We are currently seeking experienced and qualified Care Coordinators for short-term placements in aged care and community settings across rural and remote regions of Australia. This is an exciting opportunity to make a tangible impact while gaining unique professional and personal experiences. Key responsibilities include but are not limited to: Providing person-centred care and support to clients with a variety of needs, including those with dementia and high care requirements. Developing, coordinating, and implementing care plans tailored to individual client goals. Supporting clients with mobility, including safe operation of hoists and other manual handling equipment. Collaborating with families, carers, and multidisciplinary teams to ensure holistic care delivery. Maintaining accurate, timely documentation in line with organisational and regulatory standards. Promoting a safe, inclusive, and culturally respectful environment for clients and colleagues. Essential criteria: Certificate III (or higher) in Aged Care, Home and Community Care, or Disability. Qualified and experienced Care Coordinator, with a minimum of three years’ experience working in community or residential aged care. Demonstrated knowledge and/or recent experience in aged care, including dementia care and manual handling (hoist operation). Current Level II First Aid certificate. Current Australian driver’s licence, and access to a registered vehicle with a minimum of third-party insurance. Current or recent employment experience as a Care Coordinator. Current Police Check and Working with Vulnerable People (WWVP) registration, or willingness to obtain prior to placement.
